A Healthy Fight against Hunger and Disease
Greensboro, NC - Partnership for Community Care (P4CC) has been selected by the Food Lion Feeds Program to receive 41 cases of healthy food, totaling ~7,700 meals! This food will be used by the Partnership Pantry and distributed to Medicaid and Uninsured patients in Guilford County who are participating in P4CC’s Care Management program. Nurse Care Managers and Registered Dietitians work with these patients to help manage their chronic health conditions, which are often exacerbated by food insecurity and limited access to healthy food choices.

According to the pantry’s co-managers and dietitians Caitlin Romm RD LDN and Sheri Vettel MPH RD LDN, “The Partnership Pantry Healthy Food Bank Program is P4CC’s answer to the troubling issue of food insecurity in our community. Through our healthy food donations, partnered with nutrition education, cooking demos, and food budgeting, our staff is able to empower low-income individuals in our community to make the best food choices they can to manage their chronic disease(s). P4CC founded its Partnership Pantry Healthy Food Bank Program in 2013 in response to a need seen daily with patients. Many were battling food insecurity and chronic diseases, and P4CC’s Registered Dietitians sought a way to help. Guilford County is a particularly hard hit area for both poverty and poor nutrition - Greensboro-High Point was ranked #1 in the United States in food hardship in 2015 (Food and Nutrition Research Action Center). To help empower Pantry participants, food donations are paired with nutrition education provided by the Registered Dietitians.
The Food Lion partnership is part of P4CC’s National Nutrition Month celebration in March. The Food Lion Feeds Program supports hunger relief in our community, with a goal to donate 500,000,000 meals by 2020. Food Lion Feeds believes that relieving hunger is not just about eliminating the choices people make when they give up food, but helping them make the right food choices too. The donated food meets the nutrition standards of Food Lion’s ‘Guiding Stars Program’ and includes healthy pantry staples such as oatmeal, brown rice, dried beans and no-salt-added canned goods.
About Partnership for Community Care & Partnership Pantry Healthy Food Bank
Partnership for Community Care is responsible for the support of Medicaid patients in Guilford, Rockingham and Randolph Counties. We presently support 70% of all Carolina Access patients in these three counties. Our purpose is to identify and minimize barriers to healthcare by helping primary care providers in our network better utilize evidence-based disease state management guidelines. We also provide Care Management services to patients to reinforce their provider's treatment plan and to better manage their chronic conditions.
