New Name, Same Great Vision

Partnership for Health Management (P4HM) will change its name to Partnership for Community Care (P4CC) as a part of a statewide effort to brand with the statewide initiative Community Care of North Carolina (CCNC).

“This name change and awareness campaign comes at a time where we are dedicated to providing care that is patient-centered, provider driven, community based and cost effective” said Claudette Johnson, President of Partnership for Community Care. “Many people familiar with Partnership for Health Management don’t realize that we are a part of a statewide initiative to provide cooperative, coordinated care through the Medical Home model. Changing our name allows us to express the commitment we share with the other CCNC Networks to empower our communities to improve the quality of their healthcare” Johnson added.

While Partnership for Community Care will have a new name, it will still offer the same level of service, programs and commitment it has been known for since 1998. “We are changing our name to better describe what we do and visually connect us to the statewide initiative Community Care of North Carolina” said Johnson. “We will continue to offer Medicaid patients of Guilford, Randolph, and Rockingham County a comprehensive level of care to help them improve their quality of life.”

Immediate steps to launch the new branding for the company will include the introduction of the newly designed website, new logo, a Facebook page, e-mail marketing, plus branding and identity. The new website can be viewed at www.P4CommunityCare.org.

Partnership for Community Care is responsible for the support of Medicaid patients in Guilford, Rockingham and Randolph counties. We presently support 70% of all Carolina Access patients in all three counties. Our purpose is to identify and minimize barriers to healthcare by helping primary care providers in our network to better utilize evidence-based disease state management guidelines. We also provide Care Management services to patients to reinforce what their provider's treatment plan is and to better manage their chronic conditions. Currently we have approximately 85,000 enrollees in our network.

Based in Raleigh, NC, Community Care of North Carolina is changing the health care experience by changing the way health care is delivered. Through public-private partnership, CCNC has brought together regional networks of physicians, nurses, pharmacists, hospitals, health departments, social service agencies and other community organizations.
