Health Care Collaboration to Improve Care for North Carolina's Most Fragile Children
Our Children:
In North Carolina, 5% of children with special needs account for 53% of Medicaid’s child health expenditures. A significant number of these children with complex, chronic illnesses have been in a neonatal intensive care or pediatric intensive care unit and are at risk for readmission because of their fragile and complex medical state.
Here is what you should know about our children:
- Pediatric ages 0-17
- Multiple complex conditions
- Prematurity resulting in chronic conditions
- Receiving care from one or more specialty physicans
- Frequent hospitalizations or Emergency Room visits
- High costs
Benefits for Providers:
- Provider will be able to better manage the care of chronic complex children by coordination between primary care physicians and specialty care providers.
- Collaboration between previously unaffiliated providers will reduce duplication and help break down fragmentation of care.
- Improved coordination of services to high risk children is expected to lead to better outcomes, lower costs and improved patient experience.
